Clinical services

At Gradan Care our teams work closely with NHS Child and Adolescent Mental Health Services (CAMHS) to support young people in care with complex emotional and behavioural needs. Each placement includes a tailored therapeutic plan, developed in collaboration with external professionals and the registered care manager, to provide comprehensive wrap around clinical support.

Advocacy for young people

At Gradan Care, we believe children and young people should have a voice and access to independent support when they need it. That’s why we partner with the National Youth Advocacy Service (NYAS), providing each young person with an advocate to offer guidance, raise awareness, and help them express their views on important issues.

Complex needs care

Each young person receives personalized home-based tuition until they’re ready for a formal school setting in a mainstream provision. As part of Gradan Cares growth strategy we are currently exploring the set-up of our own in house alternative education provision. Our small homes range from solo occupancy , Dual occupancy and further homes will be registered, accommodating up to and no more than four young people, providing a safe space for social interaction without the pressures of larger group settings. This resilience-led approach helps young people reflect on past behaviour’s and underlying trauma allowing children the opportunity to build positively toward their future

Placement stability

In our homes, we understand that many young people have faced multiple placement breakdowns. We prioritise placement stability to provide a safe foundation where they can explore their underlying problems, providing a platform for children to express who they are and what they want to achieve. Guided by our Principle of Non-Exclusion, we carefully plan each admission, anticipate challenging behaviours, and maintain flexible resources to adapt and find creative solutions as needs evolve.

Our Homes

Our homes are detached houses set within local communities as we at Gradan Care believe in inclusion. The homes and settings vary and may provide bespoke accommodation for up to four young people in each home. Each young person has their own, individual bedroom with separate bathroom facilities with one or two properties offering en -suite bedrooms. Each young person has a dedicated key worker who meets with them on a regular basis and works closely with the therapeutic clinician. The clinician provides therapeutic support to the young people to develop, monitor and review behaviour management strategies.

Education and achievements

The organisation is in the process of creating an education service, children who live at Gradan care typically have had a negative experience when accessing formal education.  We plan to offer tailored Individual educational programs, delivered by skilled and knowledgeable teaching staff who can support to achieve the best possible outcomes for our children. We firmly believe that children who live with us have the same opportunities as any other child.  The service will offer a wide variety of nationally recognised qualifications and courses to offer further opportunities as they move into adulthood and Independence.
